Health in the school years can be pivotal in determining outcomes later in life.
School Nurses work within the Getting It Right framework as part of the multi disciplinary team around the family.
The local school nurse will be a health partner to the plan for any school age child or young person with an identified health need. They will have a specific focus on families who may be suffering the impacts of inequalities, which may affect their heath and development.
The team provide “at home” or “in school” support to children young people and families and will have a link nurse to support local residential children’s homes.
The registered school nurses in the team will be the health partners to the children at risk or are experiencing care. They are responsible for supporting planning as part of the team around the family and for ensuring health needs are met through the Child’s Plan.
Children and young people can be referred to the School Nurse by their school, their GP or other health care professionals. We always encourage professionals to contact us directly to discuss the concern or query relating to the child or young person. If we can help then we may ask you to complete a request for assistance form as this provides us with all the necessary information required to work with the child and family. If the referral is appropriate and the nurse has capacity on her/his caseload, the School Nurse will take the child onto their caseload and undertake assessment and any appropriate intervention (e.g. Decider Skills Interventions) for a time limited intervention period. We will monitor the level of referrals and trends to ensure that our nurses work at safe workload levels and to best explore the ongoing health needs of the school age population.
The child would be discharged once an intervention had been completed and the child was considered well enough to be supported via universal systems. For children will ongoing vulnerability, the School Nurse would continue to support where appropriate working with the team around the child.
Children and young people can also self refer. They can do this by discussing this at drop ins or scanning the QR code on the School Nursing Team posters to make email contact with their local team. Parents can refer by discussing with their GP or child’s teacher/ guidance teacher. Home schooled children can access the School Nursing teams by self referral or ask a professional and/or parent to make a referral on their behalf.
